Exercise is a subcategory of physical activity in which “planned, structured and repetitive movements are performed to maintain or improve fitness” (e.g. running, strength training, Pilates, yoga).60 To date, most exercise studies have focussed on the mild end of the disease spectrum (i.e. non-fibrotic/non-cirrhotic NAFLD), with changes in hepatic steatosis as the primary outcome. Physical activity is any activity that requires use of more energy than the body requires at rest, and incorporates many of the activities carried out as part of the daily routine (e.g. housework, gardening, walking up/down stairs).60 General health guidelines promote at least 150 min/week of moderate-vigorous leisure time physical activity or 10,000 steps per day for the primary prevention of CVD and to decrease the risks of metabolic diseases.59,67,68 However, most people in the general population do not reach this target, which may be contributing to the rising numbers of people affected by obesity, T2DM and NAFLD. Interruptions in sedentary time have been shown to be beneficial for weight control, glucose and triglyceride levels.66 To date, it is not clear whether a minimum duration and/or intensity of activity is required for the interruption of sedentary behaviour to have beneficial metabolic effects. Furthermore, research into targeted therapies that address specific metabolic pathways involved in IR and NAFLD is crucial. The development of non-invasive diagnostic tools, such as advanced imaging techniques or serum biomarkers, could reduce reliance on liver biopsies . Research has shown that increased intake of saturated fatty acids in the diet can lead to unfolded protein response (UPR), which in turn causes ER stress and cell apoptosis . Meanwhile, excessive intake of saturated fatty acids and cholesterol in the diet can also lead to the development of NASH. Table 1 summarizes the characteristics of the 101 patients included in this study at baseline and throughout the follow-up. To assess the association between weight loss groups and cirrhosis development, a multivariable competing risk regression model was performed. A positive value for percentage weight loss represents weight loss, whereas a negative value represents weight gain. Patients’ baseline model for end-stage liver disease-sodium (MELD-Na) and Child-Turcotte-Pugh (CTP) scores were calculated. However, more data about the therapeutic effect of SGLT-2 inhibitors on liver histology are needed. For patients on GLP-1 therapy, the coprimary endpoint of MASH resolution was achieved in 27% and 39% of patients treated with resmetirom 80 mg and resmetirom 100 mg, respectively, compared with 32% and 38% of patients not treated with GLP-1 therapy. Patients on GLP-1 therapy at baseline showed no difference in baseline MRI-PDFF or body weight. More patients who were diabetic (58.1%) and diabetic receiving GLP-1 therapy (61.3%) were being treated with a statin at baseline compared with nondiabetic patients (30.4%). Among patients with diabetes, there were no meaningful differences in baseline characteristics between patients treated with either GLP-1 or SGLT2 therapy. Fibrosis improvement was defined as at least a 1 stage improvement in fibrosis, with no worsening of NAS. Non-alcoholic fatty liver disease (NAFLD) is a chronic condition characterized by hepatic steatosis in the absence of significant alcohol consumption and is increasingly recognized as the hepatic manifestation of metabolic syndrome (MetS). TNF-alpha induces inflammation and increases insulin resistance by downregulating adiponectin. Browning et al. found that the LC diet decreased intrahepatic triglyceride content by 12%, compared with 5% in the low-calorie HC diet. On the contrary, Tendler et al. , Browning et al. , Martens et al. , and Amy et al , all found that an LC diet lowered hepatic triglyceride content more than a low-calorie diet. In addition, consumption of excessive carbohydrates results in increased hepatic de-novo lipogenesis as evident by Sevastinova et al. study where the participants were overfed with simple carbohydrates resulting in accelerated hepatic de-novo lipogenesis. The practical impact of these proposed tolerogenic roles for aHSC, which would appear to conflict with their inflammatory properties, remains uncertain. Surprisingly, several mechanisms have been identified by which activated HSCs are capable of promoting a tolerogenic environment in the liver. When exposed to low levels of LPS, aHSCs activate the NF-κB pathway, secrete pro-inflammatory cytokines including IL-8, upregulate leukocyte adhesion molecules ICAM-1 and VCAM-1 and suppress the TGFβ pseudoreceptor Bambi (30, 31). To protect against these threats, the liver recruits innate immune cells and also engages other liver cell populations including liver sinusoidal endothelial cells (LSECs), Kupffer cells (KCs), and HSCs (27). A bedside medical device is available for measuring the degree of liver stiffness (fibrosis) or liver fat content; this is known as transient elastography. Interestingly, 24.3% of this group was diagnosed with NAFLD because there was evidence of steatosis on ultrasound examination of the liver, and there were no other identified possible causes for the elevated LFT.72 Another research group in a similar setting aimed to determine the degree of fibrosis in their patients with NAFLD, as this would be consistent with a high risk of NASH diagnosis. In a retrospective analysis of 64 patients with biopsy-confirmed NASH who received bariatric surgery, 84% of patients revealed resolution of steatohepatits and 72% experienced fibrosis regression at follow-up liver biopsy 5 years later.65 Bariatric surgery has also been shown to reduce overall cardiovascular mortality in obese patients, compared with usual care.66 Weight loss results from Zepbound versus Wegovy in a month can depend on individual response, diet, and physical activity.
