In all the cohorts, patients with below-average penises were extremely rare, and most patients had normal penises.1,2 The second group of male patients seeks surgery because their penis is physiologically too small. Although most of these patients have less trouble maintaining sexual function, many still undergo surgical treatment. Even males with average-sized penises wished to have larger penises, with studies revealing that males are often dissatisfied with their penile look, which is correlated with the frequency they overestimate their penis size. These procedures exhibit a spectrum of outcomes and complications, with some showing promising results in increasing penile girth. With advances in technology, tissue engineering may be helpful in penile enlargement (Fig. 7). In a recent survey, Spyropoulos et al. conducted a study on the microscopic ultrastructural examination and surgical benefits of biodegradable scaffolds used to expand the penile circumference. Perovic et al. also performed a second penile widening among 12 patients using a biodegradable scaffold containing enlarged autologous scrotal sarcolemma cells. The Transformational Power of Telehealth: Revolutionizing Healthcare Delivery In fact, the HA concentration of the filler in this study was 23 mg/mL, which was higher than the 20 mg/mL HA concentration in previous studies, and the cross-linking degree was 12%, which was higher than the 6%–10% reported in previous studies. These technologies aim to achieve enhancements in precision and durability. Innovations such as Penuma and assistive technology for the penile tunica albuginea, along with their potential for tissue engineering, are steering the field toward more promising directions. Recognizing these challenges, the recent technological advancements and innovations in surgical techniques are promising. Given the increasing emphasis on physical appearance, “small penis anxiety” (SPA) must be recognized and treated appropriately. Four entry sites were made on the penile base and distal penile shaft at the 2 o'clock and 10 o'clock positions, respectively. The PLA filler (PowerFill; REGEN Biotech, Inc., Seoul, Korea) is a currently commercially available powder consisting of 10 g of 50-μm PLA microparticles suspended in 3 mL of methylcellulose and carboxy-methylcellulose that is reconstituted with 8 mL of saline immediately before injection. It is a biphasic HA product with a particle size of approximately 700 μm. During the study period, assessments before and after the procedure were performed by a single physician, independent of the filler injection at each institution (evaluator-blinded). However, it seems reasonable that, if possible, non-invasive techniques should be proposed as a first-line treatment. These procedures vary in complexity and require specialized training and experience. Both invasive and non-invasive techniques achieve similar improvements in penis length. This approach should be incorporated with psychological support, including counseling or therapy, to address body dysmorphic disorders or psychological distress related to genital size concerns. Papers discussing surgery in animal models and case reports were excluded from the present study unless further evaluated in a follow-up case series. The Belgian-Israeli man, who was known for visiting the clinic multiple times a year for pricey procedures, elected to continue with the surgery. One defendant told the court that the first call was made at Laniado’s insistence, with the patient claiming he was suffering from abdominal pains. Therefore, the primary purpose of this review was to conduct a literature search to evaluate the efficacy of surgical and nonsurgical treatments for penile enlargement. However, limited scientific evidence is available to support the effectiveness of these uncontrolled medicines, elongation devices, or surgical therapies in increasing penile size. Data suggest that better objective surgical outcomes are possible, though in many cases long-term data and patient-rated satisfaction details are lacking. These advancements, which are characterized by fewer complications, more effective results, shorter operation time, and higher patient satisfaction, are pivotal for the development of treatment options. Ingenuity and device innovation are vital to technological surgical advances that can produce a more sustainable improved quality of life for patients. Food and Drug Administration (FDA) in 2003, it has become the filler of choice in the dermal filler market and clinical practice . PGE for cosmetic purposes requires clarification on various aspects, such as standardized methods and performing the procedure in patients with psychiatric distress . The mean increase in girth was 22.74±12.60 mm and 20.23±8.73 mm in the HA and control groups, respectively. Penile girth, satisfaction level, Premature Ejaculation Profile (PEP), and self-estimated intravaginal ejaculation latency time (IELT) were assessed at baseline and at 24 weeks post-injection. This refined approach is essential for advancing the field and ensuring effective and patient-centric care of patients with SPA. Baseline demographic and clinical characteristics were not significantly different between the groups (Table 1). Of the 78 subjects who were screened, 4 who met the exclusion criteria were excluded; thus, 74 subjects were enrolled in the study and randomly assigned to the HA or control group. And no trusted medical organization endorses penis surgery for purely cosmetic reasons. But, there’s little scientific support for nonsurgical methods to enlarge the penis. Penis enlargement surgery, patient type, and normal penis size One of the most pervasive myths surrounding penis enlargement is the notion that bigger automatically equals better when it comes to sexual performance and partner satisfaction. Until more rigorous investigation with accurate reporting of complications is achieved, penile girth augmentation procedures should be considered experimental. Complications were reported in a minority of patients, but they may be devastating and include penile fibrosis, sexual dysfunction, device infection, and death. To increase the actual penile length, Perovic et al. described the penile disassembly technique, during which the penis is separated into the neurovascular bundle (NVB) with glans, corpora cavernosa, and urethra . These methods should be recommended in the first instance to patients with true micropenis for whom the methods previously considered could be ineffective. Zhang et al. performed a suprapubic liposuction, penile suspensory ligament release, and insertion of a folded acellular dermal matrix between the corpora cavernosa and pubis symphysis in 15 men with a buried penis. However, 47, 12, 10, 8, 7, 4, and 3 patients experienced erectile discomfort, delayed healing, insignificant enhancement, wound hematoma, foreskin edema, wound infection, and dorsal skin necrosis, respectively. These extensively utilized methods for penile enlargement demonstrate effectiveness and yield high patient satisfaction. In 2012, Kang et al. reported 52 patients who underwent phalloplasty, including autologous fat injection. The International Index of Erectile Function scores indicated that device satisfaction was generally favorable, with only 4 patients discontinuing use due to mild and reversible adverse outcomes. This study demonstrated the efficacy of the penile stretching device in increasing penile length, but significant evidence of greater penile girth was not noted. Penile augmentation surgeries have garnered attention throughout history, stemming from ancient practices to modern medical advancements. This is a subdermal implant inserted through a transverse incision above the symphysis pubis or scrotum and sutured below the glans and at the base of the penis 37,65. After being mobilized from the groin, the flap was tunneled under the pubic region to emerge at the base of the penis and then sutured to the subcoronal area and on either side of the spongiosum. Minor complications, including seroma, lumps, ecchymosis, and suture dehiscence, were resolved with conservative treatment within 3 weeks . Entry sites were made using 21-gauge or 18-gauge needles for the HA or PLA filler, respectively. The injected HA filler (Doublofill; Humedix, Co., Ltd., Anyang, Korea) is a newly developed PGE product consisting of 23 mg of cross-linked HA in 2 mL of saline. The control group was injected with polylactic acid (PLA) filler, previously approved by the KMFDS for PGE. Therefore, in this randomized controlled trial, we aimed to evaluate the efficacy and safety of PGE using HA filler and its clinical impact on ejaculation. Furthermore, PGE using filler improved clinical symptoms related to ejaculation. PGE with HA and PLA fillers resulted in significant enhancement of girth without serious AEs with no significant differences. With these advancements, penis enlargement surgery might become safer and more effective, benefiting from new technologies such as gene editing, stem cell therapy, nanotechnology, 3D printing, and mature biotissue engineering. Many men who seek enlargement treatments struggle with body dysmorphia, a condition where individuals perceive their bodies in a way that doesn’t reflect reality. For example, a 42-year-old male underwent a penile implant surgery only to later experience issues with erectile function. A ventral phalloplasty involves a vertical incision parallel to the phallus ~1 cm from the phallic edge connected to a convex curve taken from the scrotal edge of the outstretched penoscrotal web. At a mean follow-up of 12 months, 73.3% of men remain satisfied with their results and have required no further intervention. To cover the defect in the ventral shaft skin, local flaps are created by making a ventral midline scrotal incision with horizontal relaxing incisions. This day case procedure involves an initial ventral slit made in the phimotic ring and exposing the penis. During the injection, the needle was indwelled at the penile base at 1–2 and 10–11 o’clock positions with a volume range between 10 and 22 mL. Each technique is evaluated based on its clinical outcomes, durability of results, and complications. The penile length gain occurs in both flaccid and erect status and is mainly dependent on the flexibility of the NVB and urethra. Complications can be minor with hematoma and penile oedema which most commonly resolve with conservative treatment. No adverse reactions or need for a second surgery were reported, except for one case of nodular fat occurrence . The fat suctioned with a liposuction apparatus is divided into syringes after being filtered and is then injected into the penile tissue with the assistance of a cannula, thus ensuring equal distribution. Several modifications of this technique have been described regarding either the site/shape and size of tunica albuginea incisions or the closure of defects with Buck’s fascia instead of graft material 34,54,55 (techniques described in detail in Table 1). Furthermore, procedures for extending penile length can be categorized into those enhancing the perceived length and those increasing corporal length, whether or not penile prosthesis implantation is involved. Data on flaccid, stretched, and/or erect penile length, as well as patient satisfaction and quality of life, were extracted when available. Clinical evaluation and the preliminary psychiatric evaluation can help in discerning those patients who would benefit from medical therapy or minimally invasive treatments from those who would benefit from surgery 12,15. A variety of techniques have been developed for enhancing the length and girth of the penis, each with its own set of advantages and disadvantages. The rising demand for such surgeries reflects the societal emphasis on physical appearance and sexual performance. A recently developed penile implant made of silicone is a Penuma® implant and can be inserted to increase penile length in flaccid status 37,65. Postoperative complications may occur as an infection, penile skin pressure necrosis, or seroma formation, usually treated conservatively 62,63,64. Fibroblast cells harvested from biopsied scrotal dermal tissue are expanded in culture and suspended cells in culture medium are then seeded on pretreated tube-shaped PLGA scaffolds and incubated for 24 h. Penile augmentation surgery, also known as penile enhancement or penis enlargement surgery, is a surgical procedure that aims to increase the length and/or girth of the penis. The third group of male patients undergoes cosmetic penile contour surgery and repair for congenital and acquired causes of penile damage.22,26,32 These men seek various enlargement methods to increase the size and girth of their penises. Finally, penile girth enhancement may be performed using soft tissue fillers, grafting procedures, biodegradable scaffolds, and Penuma®. Despite some concerns about PGE using injectable filler in terms of patient satisfaction, a high satisfaction level with penile appearance and sexual life through a careful approach to men looking for penile enhancement was achieved in the current study. Among voluntary applicants for the clinical trial, healthy men aged between 20 and 65 years seeking cosmetic PGE and having a heterosexual and stable sexual relationship were included. This was designed as a prospective, patient/evaluator-blinded, randomized, active-controlled, multicenter trial to evaluate the efficacy, safety, and clinical impact of ejaculation following PGE using injectable filler 24 weeks after the injection. Furthermore, it is necessary to determine whether HA fillers with different properties (HA concentration and cross-linking degree) show similar effects and safety to those of previously studied HA fillers. A major advantage of HA filler over temporary fillers such as fat and collagen is its low complication rate . In addition, the repeat surgical technique resulted in a considerable improvement in peripenile enhancement, with a mean follow-up period of 38 months. This enlargement exceeded that of most previously published surgical procedures. However, penile prosthesis surgery does not affect the size of the penis but may indirectly help the penis grow larger. In 2018, Antonini et al. conducted a study of postoperative vacuum therapy after AMS™ LGX 700® inflatable penile prosthesis placement in 74 patients with medically refractory erectile dysfunction. In a 2014 study by Hsu et al., 35 patients underwent extensible penile implantation. Previous reviews have also acknowledged the lack of valid methods for evaluating outcomes, particularly in procedures for aesthetic purposes 10,67,68. The papers analyzed in this review demonstrated inconsistent approaches in evaluating changes in penile dimensions, highlighting the absence of a consensus in assessing and reporting efficacy outcomes. However, it is important to note that the scientific evidence relies heavily on studies with inadequate internal validity, such as observational designs, non-standardized methodologies, and heterogeneous populations. Conversely, more invasive methods including penile disassembly and total phalloplasty offer substantive length gains . The aforementioned studies using autologous fat augmentation and dermal fat grafting demonstrated considerable changes in the average penile circumference. This trial demonstrated that autologous fat is suitable for penile enlargement and has few adverse effects. In recent decades, autologous fat injections have been widely used for cosmetic, breast augmentation, and enhancement procedures. During the nontreatment period, significant changes in penis length and girth were not observed. Most non-surgical methods lack strong evidence, and pills or lotions are generally ineffective. The American Urological Association considers these procedures experimental for cosmetic purposes due to inconsistent outcomes. Often driven by personal insecurities or societal pressures, the decision to undergo this surgery requires careful consideration due to its risks and variable outcomes. In our view, sexological counseling and detailed explanation of risks and complications are mandatory before any operative intervention. In this article, we consider patient selection, outcome evaluation, and techniques applied. This misconception can lead to unnecessary anxiety and pressure on men to pursue enlargement methods without considering their actual effectiveness or safety. Herein, we review the data on penile size and conditions that will result in penile shortening. Consistent, long-term use of VEDs is sometimes medically prescribed for erectile dysfunction or rehabilitation after prostate surgery, but not for cosmetic enlargement. Men believe that their actual penis size is significantly smaller than the ideal size (i.e., what they think their penis should be or what they expect it to be after penis enlargement surgery). Growing dissatisfaction among males regarding their penis size is driving the popularity of penile augmentation procedures that may lengthen or widen the male reproductive organ. Therefore, this review evaluated the mental health status of men who complained of a small penis and examined the state of nonsurgical and surgical treatments for male patients seeking penile enlargement, along with a risk assessment for each. Therefore, esthetic PGE should not be performed in patients with psychiatric disorders, and a multidisciplinary approach, including psychological counseling, is necessary. Two subjects in each group reported inflammation at the site of injection that was mild and improved with conservative treatment. Ten patients dropped out (four withdrew informed consent and six were lost to follow-up), leaving 64 subjects (32 each in the HA and control groups) who completed the trial (Fig. 1). Patients were observed for 30 minutes after the PGE procedure to monitor injection site complications such as hemorrhage, pain, induration, swelling, and redness. With conservative therapy, postoperative sequelae included infections, penile skin compression necrosis, and seromas in 3, 2, and 5 cases, respectively. Biopsies of the scrotal cutaneous tissue were used to obtain fibroblasts, which were cultured until the total cell count reached at least 2 × 10. Perovic et al. performed the surgery on 204 men with a mean age of 26.77 years. Although most of these men had normal-sized penises, they tended to worry excessively and overestimate the average penises of other men. The first group of men experienced pain because they were insecure and unhappy with the size of their penis, fearing that it was not big enough to satisfy their spouses or themselves. The patients who underwent this treatment claimed various justifications for wanting to enlarge their penile circumference. Edema and congestion of the penis and scrotum were observed postoperatively, along with an area of sloughing on the dorsum of the penis, which re-epithelialized spontaneously . It was wrapped around the penis short of the corpus spongiosum and insinuated under the glans. The superficial circumflex iliac vessels were identified, and the groin flap was elevated from lateral to medial, rotated toward the penis, and tunneled into a penopubic incision. The flaps obtained were shaped with a scalpel, to perfectly fit the shapes of the openings made in the albuginea, and then sutured. Moreover, the condition can be classified as a body dysmorphic disorder if the patient experiences obsessive behaviors, significant psychological distress, and SPA present at least 1 h per day . Small penis anxiety (SPA) is a condition some men with normal-sized penises may experience, where they feel substantially distressed about the length of their penis . The growing demand for penile augmentation surgeries is driven by various factors such as increasing awareness about the availability of these procedures in combination with the availability of different techniques. In conclusion, while penile augmentation surgeries offer potential solutions for individuals concerned about genital size, the risks and complexities need to be accounted for. As a board-certified urologist, I’ve seen a growing demand for penis enlargement procedures. There aren’t enough studies of penis-enlargement surgery to give an accurate picture of risks and benefits. At best, surgery may give a slight increase in girth to the penis. All complications were treated accordingly, and no further deterioration or serious sequelae were observed. The authors attributed the decline in satisfaction to an erectile stiffness disorder because the postoperative cavernous body was covered with a softer HA. In homosexual relations, gay men predominantly achieve sexual climax through oral and anal sex. In studies regarding female orgasms, 62% thought vaginal intercourse was the best trigger for orgasm. Abstracts, opinion papers and case series ≤10 patients were not considered. The authors confirm that the data supporting the findings of this study are available within the article. In conclusion, while penile augmentation surgeries offer potential solutions for individuals concerned about genital size, they involve considerable risks and complexities. This collaboration should also be encouraged between different institutions to facilitate data-sharing and to enhance understanding of patient outcomes. Lastly, it examines interventions aimed at enhancing both the length and circumference of the penis. This therapeutic advantage is due to the steady remodeling of neovascularization, fibroblast-like proliferation, and collagen accumulation, which are characteristics of highly vascularized normal connective tissue in the adult sarcolemma. According to scientists, autologous sarcolemma cell implantation on biodegradable scaffolds is a novel and promising therapeutic strategy for a realistic increase in penile circumference. Our initial experience with this procedure revealed a significantly lower complication rate than previously established procedures. HA, calcium hydroxylapatite, polymethylmethacrylate microspheres, and PLA fillers have been examined for use in PGEs 12,17,18. This is consistent with other recent studies on PGE using HA and PLA fillers 13,14,15. There was no statistically significant difference between the groups with respect to change in the subjects' satisfaction with penile appearance compared to the baseline value at 12 and 24 weeks post-injection. The mean penile girth at baseline was 81.75±9.86 mm in the HA group and 80.37±6.79 mm in the control group (no significant difference). Only 1 enrolled patient had a medication history of phosphodiesterase-5 inhibitor for erectile dysfunction and there was no patient who had a medication history for other sexual dysfunctions. Similarly, men with inflatable penile prostheses may see a greater increase in length compared with extensible penile prostheses. General penile prosthesis implantation involves prosthesis insertion into the cavernous body of the penis. Previous studies have shown that infection-delaying coatings can reduce infections by up to 50%. The penis is then bent, and additional sharp stripping is performed to expose the surface of the suspensory ligament. An incision is made toward the pelvic process and deepened with a sharp peel through the subcutaneous tissue until a shiny white perineum can be seen. Whatever the reason and/or the medical condition driving patients to seek penile augmentation surgeries, the decision to undergo penile enhancement surgery necessitates careful consideration. Recently, Adhikari reported outcomes of ten patients operated on for penile girth augmentation using dermofat grafts and SEPA (superior external pudendal artery) flaps. Augmentation of 40% and 22% of the girth of the penis was reported in flaccidity and erection, respectively, along with improved sexual self-esteem and patient satisfaction. Austoni et al. reported an augmentation phalloplasty with bilateral saphena grafts in a case series of 39 patients with either hypoplasia of the penis or functional penile dysmorphophobia. At 18 months, the mean penile girths had significantly increased in both groups and satisfaction levels were significantly higher than those at baseline in both cohorts. Most advertised penis-enlargement methods don’t work. A penis is considered small only if it measures less than 3 inches (about 7.5 centimeters) when erect. If your penis is about 5 inches (13 cm) or longer when erect, it’s of typical size. Key search terms included penis, enhancement, enlargement, phalloplasty, reconstruction, girth, and augmentation. Indeed, in the initial feasibility study of PGE using HA filler, most patients experienced decreased tactile sensation of the shaft .
