Although vecuronium is considered an intermediate-acting muscle relaxant in children and adults, in infants younger than 1 year of age it is considered a long-acting muscle relaxant. Rocuronium appears to be the drug of choice among the intermediate-acting, nondepolarizing muscle relaxants. There is a huge box filled with slightly used tennis shoes that patients and staff are collecting for needy children and the homeless. The staff involves their patients in the process to relieve any anxiety they might have. Patients often are seen in this department three to five times before and after their procedures. It is therefore appropriate to initiate recovery from nitrous oxide anesthesia with 100% oxygen rather than less concentrated O 2 /air mixtures. Diffusion Hypoxia During recovery from anesthesia, washout of high concentrations of nitrous oxide can lower alveolar concentrations of oxygen and carbon dioxide, a phenomenon called diffusion hypoxia. Unfortunately, halving the CNS concentration of anesthetic is rarely sufficient for waking the patient. RECORDING PATIENT TRANSACTIONS The administrative medical assistant is largely responsible for recording patient transactions for the practice. Patients receiving these drugs preoperatively at thromboprophylactic doses should have the drug held for 10 to 12 hours before central neuraxial block. The GP IIb/IIIa antagonists have a shorter duration of action, thus it is recommended that abciximab should be discontinued 24 to 48 hours before central neuraxial block and eptifibitide, and tirofiban 4 to 8 hours beforehand. However, basiliximab was more recently implicated in three cases of pulmonary edema in young renal transplant patients. Because of the decrease in size of the central compartment and in systemic clearance, plasma concentrations of fentanyl and remifentanil are increased. 5 7 Gastric intramucosal pH monitoring is too cumbersome to use during surgery and in the immediate postoperative period. The supraorbital notch is easily palpated along the medial superior rim of the orbit, usually 2.5 cm from the midline. The infraorbital canal is angled slightly cephalad, while the mental canal can be entered if the needle is directed medially and slightly caudad.
