We agree that a more appropriate approach should have been applied; however, if a more appropriate approach had been applied, we believe that the impact on the results would be limited because of the small number of discontinued subjects for each treatment group and small number of differences in discontinued subjects between treatment groups in our study. For the handling of missing data for some of the secondary endpoints, the LOCF approach, which is no longer generally appropriate for clinical trials of chronic pain disease, was applied. A protocol violation was considered to have occurred if a patient met the predefined exclusion criteria after randomization (eg, gastric ulcer was malignant) or failed to follow-up within the period predefined for the assessment of the primary outcome (eg, 7 days). Two of us (H.S. and K.V.) independently extracted data and assessed the risk of bias for each of the articles. Because the degree of acid suppression required to reduce rebleeding is not known, no restrictions were applied to the frequency of boluses (they could be once daily or more often), the doses of boluses, or the route of administration (oral vs intravenous). In our opinion, the wide confidence interval for mortality limits the ability to conclude with certainty that "once daily treatment with LMWH is as effective and safe as twice daily treatment with LMWH." Furthermore, as the odds ratio confidence interval was so wide for this outcome (OR 1.14, 0.62 to 2.08), it is not possible to determine whether a clinically significant difference in mortality is seen when comparing treatments. Because this outcome takes precedence over all others in determining if one treatment is better than another, we believe that it would be important to attain as much data on it as possible. The best available evidence is presented in this systematic review but further research should be performed to elucidate whether the safety and efficacy of different LMWH compounds are comparable in a once or twice daily regimen. A meta‐analysis (van der Heijden 2000) concluded that safety and efficacy of LMWH is comparable for different compounds of LMWH used in the initial treatment of VTE. For example, a systematic review of 76 studies (mainly in cardiovascular disease) that used electronic monitoring devices found significantly higher adherence with once daily dosing compared with three times a day, though not twice daily dosing schedules.36 If nonadherence is not recognized by the prescriber, and it is wrongly assumed that a patient is treatment resistant, additional drugs may be added to the regimen which may reduce adherence further. When data were combined it could be seen that 16 (2.2%) out of a total 742 participants in the once daily treatment groups suffered a haemorrhagic event compared with 22 (2.9%) events in the 766 participants in the twice daily treatment groups. The other two studies showed a non‐significant lower risk of major haemorrhage in people treated with LMWH twice daily compared with people treated once daily (Merli 2001; Partsch 1996). All of the trials clearly stated the method of randomization, concealment for allocation, and blinding of participants, except two studies 13, 18 did not illuminate whether it was double blind or not. Five of the 7 trials reported adverse effects, including headache, osteoporosis, diarrhea, vomiting, flatulence, epigastric pain, and upper respiratory infection. One study (21) used propensity score matching for the two groups but did not report exact data on the outcome definition and relevant outcomes.
