If stem cells are artificially induced to bypass senescence mechanisms, one likely long-term consequence is the induction of tumorigenesis . Leaving aside the risks of genetic rejuvenation, other less radical approaches have been experimentally developed to enhance stem cell renewal and potency while delaying senescence. At their current stage of development, these rejuvenation strategies depend on permanent genetic modification, which carries a high risk of side effects. Stem cell senescence is a major limiting factor in tissue engineering, and several rejuvenation strategies have been devised to prevent or delay it. In ageing process, dysfunctional ECs and endothelial progenitor cells (EPCs) occur abnormal metabolism, the development into mesenchymal phenotype, vascular detachment, and myofibroblast formation, resulting in fibrosis and organ dysfunction.93 ECs and other neurovascular cells are key cells in maintaining blood-brain barrier function. Endothelial cells (ECs) and smooth muscle cells (SMCs) are critical building blocks of blood channels and are negatively impacted by premature or typical ageing processes. The authors review several studies that have used PN to treat various skin concerns, including facial wrinkles and scars. PNs are biodegradable polymers that can be used to stimulate collagen production, improve skin elasticity, and reduce the appearance of fine lines and wrinkles. An early study reported noninflammatory nodules (2–4 mm) in 12 of 94 cases using intradermal PLLA, appearing 2 to 9 months postinjection.55To address nodule formation, a clinical protocol was developed, including higher volume dilution, fewer vials per session, subcutaneous rather than dermal injections, a minimum of 6 weeks between sessions, and postinjection massage.5256A recent U.S. retrospective study across multiple centers, involving 4,483 treatments in 1,002 subjects, found that only 0.4% reported PLLA nodules,57indicating that adherence to the subcutaneous injection protocol effectively reduces nodule risk. PLA's high crystallinity results in reduced flexibility, slow biodegradation, and notable hydrophobicity.50An ex vivo study comparing human skin injections of PLLA and PCL microspheres found that PLLA exhibited limited spread after massaging, while PCL showed increased dispersion, highlighting differences in tissue integration.54PLA's properties contribute to the formation of implant nodules, a significant concern with intradermal injections, particularly noted with the initial PLLA product, Sculptra (Galderma, Lausanne, Switzerland). In the intervention group, there was an improvement in skin texture and elasticity as measured by the SkinLab USB Elasticity Module (DermaLab® Series, Cortex Technology, Hadsund, Denmark) and incisional biopsy (4.0 mm punch) on the inner forearm. Of the 14 studies that conducted a meta-analysis, it was found that several researchers used HC sourced from fish 8-12,15,16,18,19. Over time, in conjunction with reduced HA synthesis, this can result in the formation of wrinkles, dryness, and a decrease in skin elasticity . Skin elasticity is a fundamental aspect of skin health, primarily governed by the composition and structure of the dermal 21-23. Although there are a few recommendations and classifications proposed for the adoption of reporting important characteristics of the PRP preparation in the scientific literature,45–47 as evident from our systematic review, none of these are implemented at large. Moreover, there is no standard reporting system for PRP to deduce and replicate the results of the published literature. However, post-inflammatory pigmentation was reported as a side effect of the treatment.39 Patients’ objective scores also increased significantly in combination therapy suggestive of the synergic effect of the treatment. After treatments, all patients were instructed to cleanse their face with a gentle skin cleanser (Avene Tolerance Extreme Cleansing Lotion; Avene in Avene, France) and to use sunscreen daily (Cyberderm Sun Whip SPF 25; Cyberderm, Ottawa, Ontario). Skin tightening and collagen formation continues for 5 to 7 years postprocedure.10 In this study, we analyze the effects of microneedling with and without a novel topically-applied growth factor. For example, copycat cryolipolysis systems have caused burns and freeze injuries63 while some HIFU systems have caused corneal stromal damage.64 Without the exacting and rigorous standards of clinical studies, it is difficult to establish clear guidance to prevent improper techniques or untested protocols. The transcriptional activation program of mitochondrial chaperone proteins and proteases is known as the mitochondrial unfolded protein response (UPRmt), which is a mitochondrial response to stress.139 Studies have shown that UPRmt plays a significant role in many physiological processes and that its activation increases longevity and prevents ageing by regulating mitochondrial proteostasis.140 During mitochondrial dysfunction, there are many transcription factors necessary for the activation of UPRmt genes in mammals. Based on our analysis, the FSH levels decreased significantly in all three months they were examined after PRP treatment. Most authors have not examined PRP treatment in a time interval longer than three months; therefore, there is no information on how long the positive effect of PRP treatment lasts. Regardless, it is a significant result that in the case of AMH, while the hormone level increased significantly in all three months, the highest increase was registered in the third month, thus providing a clue for the optimal timing of starting reproductive treatment. Consequently, the comparison of individual months can only be interpreted with caution, and this comparison alone is not suitable for determining the ideal start time of the IVF cycle after PRP treatment. In our meta-analysis, we examined the change of hormone levels one, two, and three months after PRP treatment of the ovary. To achieve epigenetic rejuvenation via reprogramming factors, studies must first look at the intermediate states during dedifferentiation, where cells have started to epigenetically change (presumably de-age), but have not yet fully dedifferentiated 94, 100. The developed strategies include stem cells and related EVs,446 pharmacological activation of autophagy,447 maintenance of circadian rhythmicity on skin-resided stem cell448, and regulation of metabolism homeostasis to restore mitochondrial integrity and metabolic output.449 Identifying key molecules involved in skin ageing makes great significance to skin rejuvenation. The test cosmetic and device were applied for 5 consecutive days, followed by a 2-day rest for the first 2 months of treatment and three times per week in the 3rd month. The main ingredients of the anti-aging cosmetics and gels are shown in Table 1. This study was approved by the Biomedical Ethics Committee of West China Hospital, Sichuan University. Thirty-two participants completed the study, while one withdrew for personal reasons. These two sensors improve the safety of the user and avoid skin burns due to overheating . By contrast, Lacarrubba et al.9 treated 20 women with signs of photoaging on the dorsum of the hands with hyaluronic acid salts (of biotechnological origin) and multivitamin injections (weekly for four weeks). Importantly, improvements to tissue functionality have also been demonstrated in the intervertebral disc, dentate gyrus, kidney, bone, and lung (Alle et al., 2022; Cheng et al., 2022; Rodríguez‐Matellán et al., 2020). Other individual examples of enhanced regeneration have been noted in several tissues including the liver, optic nerve, heart, and pancreas (Chen et al., 2021; Hishida et al., 2022; Lu et al., 2020; Ocampo, Reddy, Martinez‐Redondo, et al., 2016). These results support IVPR as a viable therapeutic route to accelerate muscle repair and function albeit with method and model dependent effects. In contrast, muscle regeneration and function was improved in a myofiber specific but not a satellite cell specific transgenic model of 4 factor IVPR in adult wildtype mice (12–15 month; Wang et al., 2021). The ECM is mainly composed of type I collagen produced by fibroblasts and determines the firmness and strength of the skin. Such iPSC-derived products include iPSC-MSCs, iPSC-derived keratinocytes, endothelial cells (ECs), as well as growth factors. In disease conditions with complications, such as chronic non-healing of cutaneous wounds, application of primary adult stem cells is often challenged by the small number of cells and invasive methods of obtaining them. Some authors emphasize the importance of distinct wavelengths for optimal results.16–18,26–28 In our study, the differences between the RLT and ELT treatments in clinical outcome and patient satisfaction were not significant, indicating that despite spectral differences, both light sources were commensurably effective regarding study objectives. Because fibroblasts are responsible for collagen production in wound healing, dermal remodeling, and tissue repair, we decided to focus on increased collagen density as a surrogate marker for fibroblast activity, and abandoned such invasive monitoring methods as histologic examinations following skin biopsies for our study. We investigated the safety and efficacy of a novel non-thermal, non-ablative, atraumatic, polychromatic low-level light treatment modality with a focus on pleasant skin feeling, improved skin appearance, intradermal collagen increase, and the visible reduction of fine lines and wrinkles in a prospective, randomized, controlled trial that consisted of 136 volunteers. Somatic cell nuclear transfer
Lu et al. (2020) performed an intra‐ocular injection with an AAV vector containing 3 factors; Oct 4, Sox 2, and Klf4 (OSK) and observed a decrease in DNA methylation age based on ribosomal DNA in retinal ganglion cells from 12‐month‐old wildtype mice after 4 weeks of continuous reprogramming. In order to validate future clinical development, here we review the key partial reprogramming studies, both in vivo and in vitro, in detail as related to lifespan, epigenetic age, hallmarks of aging, and regenerative effects (Figure 2). We will assess clinical heterogeneity by comparing the distribution of important participant factors between trials (e.g. age; skin damage severity at baseline; the classification system used, e.g. Glogau Classification or Rubin System, or both; treatment dose; frequency of treatment). The most common side effects were mild redness and swelling, which resolved within a few days (Level IIa). However, the combination therapy showed greater improvements in wrinkle depth and skin elasticity compared to MRF alone. Nevertheless, clinical studies are not numerous, and protocols are varied, making it difficult to draw conclusions. This suggests a balance between collagen degradation by MMP-1 and its production, facilitated by TIMP-1, enabling sustained dermal collagen production. Mice lacking AQP3 exhibit dry skin and reduced stratum corneum hydration,88primarily due to impaired glycerol transport rather than water movement, a phenomenon also confirmed in human skin.89Glycerol is nonimmunogenic and has been safely used in clinical settings for conditions like increased intracranial pressure, establishing its safety profile when injected intravenously.90 Schematic representation of the proposed mechanisms through which poly-(lactic acid) enhances collagen synthesis in the dermis. Macrophage reactions to biostimulatory substances are critical in fibroblast activity and collagen production. PDO showed a more significant increase in Col1α1, Col3α1, TGF-β2, and TGF-β3 than PLLA, whereas PLLA had a higher surge in TGF-β1, indicating its potential advantage in early atrophic scar treatment.80

In 2005, heterochronic parabiosis was initially demonstrated to reverse the age-dependent loss in stem cell function by restoring the proliferation and regeneration capacity of aged satellite cells and hepatic progenitor cells in naturally aged mice.372 Since then, heterochronic parabiosis has been used as an experimental system to examine potential systemic influences on lifespan, age-related diseases, and the process of organismal ageing. Preclinical results have shown that proT cells could effectively engraft involuted ageing thymuses, achieve rapid long-term thymic reconstitution and accelerate T cell recovery.364 The process is regulated by receptor activator of NF-B (RANK) and receptor activator of NF-B-ligand (RANKL) interactions, which generate chemokine-rich niches within the cortex and medulla that probably encourage the recruitment of bone marrow-derived thymus seeding progenitors.365 ProT cells are limited to the recipient’s major histocompatibility complex (MHC) and produce host-tolerant T lymphocytes after undergoing positive and negative selection in the host thymus. HSCs ageing is accompanied by alterations at the gene level, and genetic modulators by ablation of genes driving ageing or overexpression of rejuvenative genes might be strategies to prevent HSCs dysfunction, such as the deletion of the imprinted gene Grb10 or forced expression of Satb1.90 In addition, pharmacological intervention based on the pathophysiology of HSCs senescence also can promote rejuvenation of the HSC compartment, reconstitute the immune system activity and finally even increase overall lifespan. Ageing HSCs have less self-renewal activity, fewer cell divisions, decreased homing efficiency and myeloid lineage-biased differentiation as well as reduced output of lymphoid progenitors.360 HSCs play a crucial role in the immune system, hence functional restoration of ageing HSCs has great significance to immune rejuvenation and organismal health. A recent clinical trial reported implantation of iPSC-derived dopaminergic progenitors into the putamen (left hemisphere followed by the right hemisphere) of a patient with PD indeed improved clinical symptoms of PD after surgery.326 Reprogramming-derived stem cells are highly amenable to genetic correction and homogeneous than isolated adult stem cells, but there still exist major obstacles limiting the application, including low reprogramming efficacy and potential security concern. These cells have the potential to restore neural plasticity and replenish lost or damaged neurons, offering hope for regeneration in affected brain regions. Future research in age reprogramming holds the promise of addressing critical challenges in aging and advancing healthcare into a new era. Integrating advanced longevity treatments into these healthcare systems may prove financially and logistically overwhelming, further marginalizing populations that already struggle with basic medical care. The concomitant dehydration of tissue in the coagulation zone enhances the penetration of hydrophilic compounds, as osmotic flux into the skin is increased. Preclinical and proof of concept studies have demonstrated mechanistic justification for clinical findings. Recurrence occurred an average of seven months and up to one year after treatment and all treatment sites showed similar effectiveness. Findings were promising with 7/8 patients having a good or partial response at 6 to 8 weeks after treatment and significantly improved Physician Global Assessment (PGA) and Dermatology Life Quality Index (DLQI) scores. Patients reported high satisfaction and low VAS pain scores (average 3.3), though half of patients had prolonged erythema lasting almost two weeks. Physicians should tailor their strategies to each patient's age and clinical conditions. Although a treatment interval of 12 to 18 months is effective for maintenance,43 this can be adjusted according to age. The individualized aging pathology should also be assessed before treatment. Patient customizations should include demographic considerations, such as age, body mass index, the number of treatment lines, energy settings, and treatment depths;43 the treatment strategy, as determined by the patient’s goal(s) of restoration, prevention, or maintenance; and safety considerations.
