In 2012, Keating et al. performed a meta-analysis of 12 trials (11 of them randomized) investigating the effect of exercise on liver fat content. Another study evaluated 120 candidates for living liver donation who were encouraged to do aerobic exercise and restrict their calorie intake to 25 cal/kg, after liver biopsy showed at least 30% steatosis. A small number of studies have used postintervention liver biopsy to evaluate the effect of exercise on the histologic reversibility of NASH. Here we aim to review the existing evidence for the effects of physical exercise on NAFLD, as well as the mechanistic principles that have been elucidated through human trials and basic scientific studies. Unfortunately, greater than 50% of patients included in clinical trials have not been able to meet this weight loss threshold8.
