Based on the search strategy, a total of 16 studies were included in the review 17–31. A total of 1657 articles were identified through the electronic databases and the manual review of the reference lists of the included studies. PDE5 is the enzyme primarily responsible for the hydrolysis of cGMP in the penis.30 Inhibition of PDE5 leads to persistent vasodilation and maintenance of penile blood flow by sustaining a high level of cGMP in the cavernous tissues and arteries.30 It is important to note that PDE5I do not spontaneously produce vasodilation; sexual stimulation is required for potentiation of erection with PDE5I. Routine-dose tadalafil is also under investigation for a number of non-ED conditions;26–28 however, in the interest of conciseness, we will focus our attention on indications related to erectile function. Study selection In the treatment of PE, regarding the effect of PDE5 inhibitors, there are several mechanisms involved. Our results are supported by Ozcan et al. (17) who demonstrated that tadalafil 5mg alone could significantly prolong IELT. In this study it was reported that IELT increased approximately 2.5 min while in our study increased 1.2 min. One would not expect men who have undergone bilateral nerve resection to respond to sildenafil, though a 10% potency rate was usually quoted prior to description of the nerve-sparing prostatectomy. Because intact neuromuscular connections are required for sildenafil to work, men must have a subtotal nerve injury in order for sildenafil to be effective. This oversight is particularly worrisome considering the dramatic response rates to sildenafil in these individuals who have recently undergone an event that drastically worsens their quality of life. Two of these men were already known to have orthostatic hypotension and were receiving treatment with ephedrine and midodrine, but the third had asymptomatic hypotension. Of note, partners were prescreened using the Female Sexual Function Index (FSFI); partners with FSFI total scores less than 26.55 (suggestive of high risk of female sexual dysfunction) were excluded from the subsequent analysis. Among men with IIEF-EF scores less than 26 at baseline, normalization of IIEF-EF domain scores was attained at the conclusion of the study in 21.2% (24/113), 34.2% (40/117), 42.5% (51/120), 40.0% (46/115), and 14.0% (16/114) of men in the 2.5 mg, 5 mg, 10 mg, 20 mg, and placebo groups, respectively, with no explicit comment made on statistical significance in this study. Tadalafil was found to be significantly superior to placebo at both dosages for all primary efficacy end points; specifically, the mean IIEF-EF domain score increased by 4.8 and 4.5 points in the tadalafil 2.5 and 5 mg arms, respectively, relative to a 1.3 point increase in the placebo arm.
