However, it consistently improves strength, fitness, and body composition.27 In other words, the hormonal shifts from exercise are not the primary goal—they are part of the pathway toward better body composition, strength, and metabolic health. A 2021 meta-analysis found that in healthy men, structured resistance training does not significantly raise resting total or free testosterone. Fatigue is a common symptom of low testosterone because muscles lack sufficient testosterone to process energy in the body. High-intensity interval training (HIIT) is a time-efficient strategy to achieve health (3) and performance (4) benefits in younger cohorts, which contradicts the recommended minimum physical activity threshold guidelines (150 min/week) (5). The finding of our study shows that 12 weeks of anaerobic exercise training based on the ACSM FITT protocol is found to be effective in the improvement of estradiol level and BMD in postmenopausal osteoporotic female. The study was unique in its approach as exercises based on the prescribed criteria of FITT as per the guidelines of the American College of Sports Medicine, 2013, were used to quantify the dosage of exercises. Our results of the 12 weeks randomized control trial study suggest that exercise intervention based on aerobic and anaerobic exercise does improve the level of circulating estrogen in the overweight postmenopausal osteoporotic women. P-values of fat mass and muscle mass among the postmenopausal osteoporotic women after 12-week intervention are elaborated in Table 7. It was demonstrated that regular participation in the research experiment significantly increased the values recorded during testosterone measurements. However, given the extensive evidence of the anabolic effects of T, coupled with the known association of low muscle mass and all-cause mortality in men , these findings may have important implications for men with reduced muscle mass resulting from their inability to exercise due to disease or disability. Similarly, exercise training has been shown to stimulate muscle hypertrophy in middle and older aged men 37–41, although these studies also used some dietary manipulations. Although there is a body of literature concerning the effects of anabolic androgenic steroid (AAS) abuse, this review focuses on (physiological) T treatment, designed to increase T levels within the physiological range in middle and older aged men. A few additional studies have combined other types of exercises with resistive strengthening exercise and examined their effects on serum testosterone. Therefore, the present study aimed to investigate relationships between serum testosterone levels and body composition and physical fitness parameters in patients with erectile dysfunction (ED). To determine the best exercise program, it is important to first understand which body composition and physical fitness parameters are most closely related to serum testosterone levels, and then introduce exercise programs targeting these factors. However, it is important to note that lean mass represents the aggregate sum of any fat- and bone-free mass and may not fully reflect muscle mass. However, research has shown that females achieve relatively similar degrees of hypertrophy (57) and potentially even greater strength following RET (20). Finally, Morton and colleagues (13) trained 49 males where training was performed with either higher (8–10 repetitions/set) or lower (20–25 repetitions/set) loads. The hormonal response with the basal hormonal and HH protocols was similar pretraining to posttraining, suggesting that the hormonal response was maintained chronically throughout the training intervention. A subsequent investigation had male participants unilaterally train one arm under either the HH or basal hormone milieu for 15 wk (16). However, there have been proposals (26) that progesterone can antagonize estrogen’s effects by competing for receptors and blocking the action of estrogen (Fig. 3). Randomized controlled trials (RCTs) were included if they compared any type of exercise intervention to no intervention or other interventions, and assessed the effects on estrogens, androgens or the sex hormone binding globulin (SHBG) in cancer-free women. In conclusion, because preconditioning exercise increased both TT and SHBG, only a small increase in free-T was observed, which did not reach significance. Therefore, the confirmation that exercise can increase serum testosterone is important for medical practitioners because exercise has been proposed as an initial treatment for low testosterone (26). That previously sedentary aging males can increase TT following moderate exercise training, and free-T following HIIT, is an encouraging finding. It is worth noting that in this review, we will discuss research pertaining to biological sex, which refers to biological differences between females and males, including chromosomes, sex organs, and endogenous sex hormone profiles (40) herein referred to as male/female. Notably, we highlight responses to endogenous hormones and not exogenously administered hormones, especially androgenic steroids, of which supraphysiological (well beyond normal diurnal variation) doses are markedly anabolic and hypertrophy-promoting (39). However, current evidence indicates that menstrual cycle phase does not influence acute strength performance or RET-induced hypertrophic adaptations (26). Although this preservation was statistically significant, the magnitude (~400 g) falls within repeat-scan error for dual x-ray absorptiometry in reporting lean mass (30–32), leaving its functional (i.e., performance) significance unclear. For instance, Sites et al. (29) conducted a 2-yr study involving 76 postmenopausal females who were randomized to receive either conjugated estrogens plus medroxygenprogesterone acetate or a placebo daily. The mechanism for such a change appears consistent with earlier work supporting the development of hypogonadotropic hypogonadism as with extensive caloric deficient, weight loss and restricted food intake (see prior discussions). Interestingly, earlier researchers had drawn an analogy between the development of menstrual disruptions in exercising women and the observation of low testosterone in men but had never applied the Triad terminology to men (111, 112).
